Everett,WA(January 2, 2020) -CommunityHealthCenterofSnohomishCounty

(CHC) reported today that personal and Dental information concerning its patients

was accessed by an unauthorized individual. On November 5, 2019, a patient of

CHC visited one of CHC’s clinic locations to obtain copies of their personal records.

Due to human error, the incorrect records, which contained information relating to

patient treatment plans, were inadvertently printed out and provided to this patient.

The information, although containing very limited content or detail, did include

patients’ ­rst and last name, date of birth, medical record number (an identi­cation

number created and used only by CHC), expected dental treatment and treatment

provider. To date, there is no indication that any information potentially accessed

was misused during this incident.


Upon learning of the disclosure, CHC immediately took steps to contact the

individual in possession of the patient records to ensure their destruction or

return. To date, this individual has refused to return the records or con­rm their

destruction. Due to the nature of these records, CHC is unable to replicate the

exact list of patients affected in this incident. Therefore, out of an abundance of

caution and so that patients may diligently monitor their accounts, CHC will provide

information on how to protect your credit to those individuals whose information

was potentially accessed. CHC takes patient privacy very seriously and has

established stringent protocols to ensure patients’ information remain private. CHC

has taken every step necessary to address the incident, including conducting staff

training to ensure this incident will not be repeated.


CHC recommends patients remain vigilant in regularly reviewing and monitoring

all account statements and credit history to guard against any unauthorized

transactions or activity. Individuals may obtain one (1) free credit report per year by

visiting or calling 1-877-322-8228. If any suspicious

or unusual activity is discovered, please contact your ­nancial institution. For
